Description

T Mix (T3-T4 mix) is a combination of thyroid hormones that includes triiodothyronine (T3) and thyroxine (T4), the only biologically active substances known to contain iodine. These hormones perform two fundamental functions:

  • during growth, they are essential for the normal development of the body, particularly the central nervous system;

  • In adults, they regulate energy metabolism, affecting almost all organs and tissues, the rate of energy consumption, and heat production.

The thyroid gland maintains large reserves of thyroid hormones. Their metabolism occurs mainly in the liver, but also in target tissues such as the brain. Plasma levels of T3 and T4 are precisely regulated by the pituitary gland via TSH, according to a negative feedback mechanism. T4 is the main hormone secreted by the thyroid and is converted into T3, the most metabolically active form, especially in peripheral tissues.

Thyroid hormones act primarily through intracellular receptors, modulating gene expression and influencing numerous metabolic processes.


Purpose of use

Many patients undergoing replacement therapy have difficulty converting levothyroxine (T4) to triiodothyronine (T3), even though the body needs both hormones. Causes may include hepatic overload, zinc deficiency, nutritional deficiencies, or excess iodine.

Due to the limitations of T4 monotherapy, patient dissatisfaction remains high. For this reason, numerous studies have analyzed the benefits of combined T3 + T4 therapy. The T3-T4 mix allows T4 and TSH levels to be normalized and T3 to be maintained at least in the mid-normal range.

T Mix is used in replacement therapy for various conditions, including:

  • post-thyroidectomy

  • radioiodine therapy

  • thyroid cancer

  • suppressive therapy

It is indicated forhypothyroidism of any etiology, including:

  • primary and secondary hypothyroidism

  • diffuse or mixed euthyroid goiter

  • subacute and autoimmune thyroiditis

  • thyroid cancer (post-surgical)

  • obesity associated with hypothyroidism


Dosage

Treatment with T Mix generally begins with low doses, which are gradually increased to the therapeutic dose over a period of 3–4 weeks.

In elderly patients or those with cardiovascular disease, doses should be reduced to limit the risk of cardiac side effects. During pregnancy and childhood, the need for thyroid hormones may increase. In some cases, beta-blockers may be prescribed to improve tolerance to therapy.


How to use

Generally:

  • Start with 1 tablet per day.

  • every 5–6 days, increase by 1 tablet

  • The maximum dose is 3 tablets per day, divided into several doses.

The standard cycle lasts 5–7 weeks, followed by a break of at least 6 weeks. This approach can help restore physiological thyroid function. The T3-T4 mix is sometimes combined with clenbuterol to enhance the fat-burning effect.

  • Average dosage: 1–3 tablets per day

  • Effective scheme: pyramid method (1-2-3-3-2-1)
