Description

Raloxifene is a selective estrogen receptor modulator (SERM) that is used both in medicine and, in some contexts, by athletes and bodybuilders. Thanks to its selective action on tissues, raloxifene is sometimes used to counteract the negative effects of estrogen, such as gynecomastia and water retention.

In bodybuilding, raloxifene can be used during or after cycles of anabolic steroids, as these substances increase not only testosterone but also estrogen levels, promoting estrogen-dependent side effects. Raloxifene works by blocking the activity of estrogen in specific tissues, such as breast tissue, while maintaining beneficial effects in other areas, such as bone.

Use in medicine

In clinical settings, estrogen hormone therapy is often used to prevent and treat conditions related to women's health, such as postmenopausal osteoporosis. However, raloxifene represents a selective alternative, capable of exerting:

  • estrogenic agonist action on bone and lipid metabolism

  • antagonistic action on breast tissue and endometrium

This tissue selectivity allows raloxifene to offer estrogen-like benefits while reducing some of the side effects typical of traditional estrogen therapy. The most common side effects include hot flashes and leg cramps, while an increased risk of thromboembolism has been documented.

Benefits of raloxifene

The main beneficial properties of raloxifene include:

  • reduction in total cholesterol and LDL cholesterol

  • increase in bone mineral density

  • prevention of bone loss

For these reasons, raloxifene has been approved by the FDA for the prevention of osteoporosis in postmenopausal women.

Raloxifene vs. Tamoxifen

Both raloxifene and tamoxifen belong to the SERM class and share some indications, but they have significant differences:

  1. mechanism of action Tamoxifen blocks estrogen mainly in breast tissue. Raloxifene, on the other hand, exerts a more selective action, acting as an antagonist in some tissues and an agonist in others.

  2. usage profile Tamoxifen is often preferred in oncology, while raloxifene is more commonly used in the prevention of osteoporosis and for the management of unwanted estrogenic effects.

  3. cost Tamoxifen is generally less expensive, although prices may vary depending on dosage and market.

The choice between the two depends on individual needs and the clinical context.

Effects of raloxifene on bones

Raloxifene binds to estrogen receptors in bone tissue, stimulating new bone formation and slowing bone resorption. It is particularly indicated for the treatment of osteoporosis in postmenopausal women.

The MORE study, conducted on over 7,700 women, demonstrated a 30–50% reduction in the risk of vertebral fractures compared to placebo. Further studies have shown a significant increase in bone mineral density (BMD) in the hip and spine after three years of treatment.

Effects on lipids

Raloxifene has shown a positive impact on the lipid profile:

  • reduction in LDL cholesterol ("bad cholesterol")

  • increase in HDL cholesterol ("good cholesterol")

These effects contribute to an improvement in cardiovascular risk, although they do not make raloxifene a first-line drug for the treatment of hypercholesterolemia.

Dosage

The commonly used dosage of raloxifene generally ranges from 30 to 120 mg per day, depending on the therapeutic goal and individual response.

Main effects

  • selective blocking of estrogen receptors in breast tissue

  • prevention of estrogen-induced gynecomastia

  • support for bone health and increased mineral density

  • improvement in lipid profile

  • possible reduction in cardiovascular risk

  • potential cognitive benefits (under investigation)

Side effects

The most common side effects associated with raloxifene include:

  • hot flashes

  • leg cramps

  • nausea

  • increased risk of thrombosis

  • possible increased risk of cataracts

Contraindications

Raloxifene is contraindicated in individuals with:

  • history of thrombosis, stroke, or thromboembolic events

  • liver disorders or alterations in liver function

  • hypersensitivity to the active substance or to any of the excipients
